These templates are PDF files. Download the file, highlight the fields if they are not
already highlighted, and type in your lodge name and number. (Do not use the octothorpe
or hash tag.) A stub for record keeping is included in the “five-up” artwork, with crop
and perforation marks. There is also a field for entering a consecutive card number
although continuous numbering will require specialized software, or “crash printing” by
a professional printshop.
This artwork can be printed on your home printer or supplied to a professional
printshop. Consult with your printer regarding their requirements, especially if you
wish to include your lodge logo or different typography. If this PDF cannot be used, the
design and layout is provided as a style guide.
